Is this a standard applied across the U.S. and what is the size of a "lot" or "block"? How does the system grid where nature does not cooperate (rivers, hills, etc.)?

There is not really a "standard system" and to accomdations things like rivers and hills were simply made at the time. Many U.S. cities have many new listings of their street the late 19th century or early 20th century to accommodate growth.
